If the Media Creation Tool (MCT) can create a bootable USB drive including the Windows.iso, of what value is downloading the Windows.iso separately and then using Rufus to put it on a bootable USB drive?

    Rufus question:

My system Bios is set UEFI with CSM enabled (so I can see USB devices as alternate boot devices) and GPT disks.  When I insert a USB stick and open Rufus on my system, Rufus selects the build to be GPT partition and UEFI. The only way to get UEFI with CSM is to change the partition to MBR.

If I create the USB stick with the first Rufus selection will the MCT install in UEFI mode on my system as set up – UEFI with CSM?  Or, would I need to change the Bios to UEFI only?  And if so, how will I be able to see the USB stick as an alternate boot device without CMS?

I would say that Rufus would not be relevant to this specific situation.

Rufus scores in being able to create a bootable USB Flash Drive directly from an .img.gz file (example easy-1.0.14-amd64.img.gz) without have to mess about unGZipping it.

With regard to your further questions, I try to avoid UEFI…!

I would just use the MCT.

You install Windows to UEFI with GPT, or install Windows to legacy BIOS with MBR.

Bearing in mind the MCT also supplies the most recent iso deemed stable by MSFT, (that’s not to say the most current though) whereas using the 3rd party user-agent trick or non MSFT OS to download the iso supplies the initial release (initial bugs and all hence having to patch up)
It’s down to your own preference really.

      CSM is a machine setting.
Rufus only offers UEFI because that is how it will boot on your machine.

I used MCT just a couple days ago to create the bootable USB drive plus downloaded the .iso file for use in the creation of the bootable DVD+R/DL.  The USB is 16GB as getting both the x86 and x64 versions requires minimum 8GB but I like a little ‘leg room’.  The DL disc is also required for the same reason.   The resultant install gets Windows 10 Version 1809 Build 17763.253 if not connected to the Internet.

The biggest difference is that Rufus 3.5 can download an ISO for versions older than 1809 – useful for when you want to avoid the current release.  And it does so by running a script that grabs an authentic ISO from Microsoft’s “hidden” download servers, not some third party file sharing source.
