A fair no. of websites I’m visiting are displaying the error as shown in the screenshot.
Seems to be from some Java Site
Check the login settings under Tools>Internet Options… security tab. You first need to highlight the icon for the Internet zone (assuming this site is in your Internet zone, or whatever the relevant zone is), then click Custom Settings…, then scroll to the bottom of the list. Does it say “prompt for username and password”?
I’ve never seen that dialog before. Also, the setting I mentioned doesn’t seem to have any effect on the three pages I checked at easily.co.uk. My revised thought is that the poster’s organization is blocking selected types of active content, and that is why we can’t replicate the error.

Just looking at the controls in the dialog, it certainly looks like a Java-generated dialog. It is not using the standard Windows controls to create the buttons and form boxes. I think I would agree that it has something to do with a local firewall or similar security setting. I have seen similar prompts from proxy servers on internal networks, though not just for pages containing Java.
