    I have attached a sample excel worksheet. Could you please show me how to create a vlookup formula.

      See the attached version. The formula in A13 is

      =VLOOKUP(A12,$A$3:$D$7,2,FALSE)

      meaning: look for the value of A12 (“Susan”) in the range A3:D7, return the value from the 2nd column, and look for an exact match (FALSE = do not approximate). Similar for A14 and A15, only the column number is different.
