TOC3 showing as Hyperlink (2002 SP3)

The attached picure shows examples of how my TOC!, TOC2 and TOC3 styles show.

When I use Reveal formatting they all say that they should be blue and underlined. However only TOC3 actually shows as blue and underlined.

Does anyone know why 1 and 2 show as black text with no underline whilst 3 does not

And also how I convince TOC3 to stay as Black text with no underlining?

I can’t say for sure. It may help if you can post a sample by deleting everything but one each of the first three levels and a TOC created from them?
