Lookup and add to cell

I have over 8500 purged charts in boxes. They purged different years so John Smith maybe in many different boxes
The original Database (Black font in the attachment that i sent) did not have the DOB and SS #
The New Database which contains more names then I need has the DOB and SS # but not the box #
How can I add the DOB and the SS # to the names that has box #’S ?
Maybe use Access some how or some kind of a lookup? any ideas? Please help

You could use lookups like this. Note that the first record number for John Smith does not match the others.
