    When I select a file and then insert it into content, sometimes the wrong image is inserted.  It only works if I attach the file first, submit and then insert.

      You shouldn’t have to submit first. You should be able to insert it while replying.
      Using the “Visual Tab,” choose Select File
      Attach
      Choose “Insert into content”
      It will look like this until you Submit:

      Before pressing the Select File button make sure your cursor is positioned where you want the image inserted. After inserting you can delete some of the extra lines to neaten up your post.
