Header & Footer greyed out (2003)

Sometimes when I open a Word document that has a header and footer I cannot double click it and the Header & Footer option under the View menu is greyed out. I then have to open another Word document to get the Header & Footer functionality back again. Any idea what could be causing this behaviour?

If you open another document, then switch back to the problem document, is View | Header and Footer enabled then?

Note: Header and Footer view is always disabled in a document protected for forms, even in unprotected sections.
