Chapter numbering not appearing (Word 2007)

I have a document for which I cannot get the chapter number to appear next to the page number. I have attached a stripped down version of the document that still shows the same characteristics.

Please help!!

When I open your document in Word 2002, I do see the chapter number with the page number. The screenshot below shows the footer for chapter 2 (magnified at 200%)

As best I can tell, this document was produced from a document template originally created in Word 2000, edited most recently in Word 2003, and I am viewing them in Word 2007.

Since we cannot go back to Word 2000 or even Word 2002, what would you suggest to make this work in Word 2007?

This appears to be caused by the Frame that is part of the Heading 1 style. If you alter the style to remove the frame and then change the paragraph indents to get the same position on the page then the page numbers will work as expected. If you move the first line far enough to the left then it won’t appear on the page as per your frame layout.

Does Word 2007 have the Page Number Formatting dialog where the style associated with the chapter number (and the hyphen separator) are assigned? Can you see whether this information is accessible in the “upconverted” document?

Everything looks right in the Page Numbering Formatting dialog box. It just doesn’t work in the footer.
